Once you have completed the below form, we will confirm your policy purchase in writing via your preferred document delivery method, or if required, call you to discuss your cover.

Please note – if you do not give us permission to start your policy automatically you will be required to provide us with a start date, failure to do so will mean your cover will not start
Please note – All monthly instalments includes a 12% charge by Premium Credit Ltd as part of a credit agreement, so it does cost more overall by paying monthly. The option to pay monthly is subject to status and a Credit search may be performed by Premium Credit. We store the sort code and account number on our system, payments will only start when we acquire a start date for the policy from either yourself, your local branch/mortgage advisor, if you have consented to this above. Your first payment will be taken within 14 days of start date. When we are aware of the start date, we pass Premium Credit your details including your bank details and they will contact you via e-mail and text, you will be required to sign and return a credit agreement, however payments do not depend on this but it is required. Payments are protected by the direct debit guarantee, details of this are within your documents. Confirm you are happy with the above by providing your chosen sort code and account number for your monthly instalments below. Annual Payment Important Notice – by choosing to pay annually, we are unable to start your policy unless we make contact with you to collect payment. In the event we are unable to contact you to collect payment, we can start your policy on a monthly instalment basis automatically. Confirm you are happy for us to do this by providing your chosen sort code and account number for your monthly instalments below. If you do not wish us to start your cover on a monthly instalment basis, please leave the sort code and account fields below blank.
